Саркоматоидная карцинома пищевода – редкое злокачественное новообразование, составляющее примерно 2 % всех карцином пищевода. Ее также называют карциносаркомой, псевдосаркомой, псевдосаркоматозным плоскоклеточным раком, веретеноклеточным раком и полипоидным раком, что отражает неопределенность его патогенеза. В статье представлен случай у пациентки 68 лет с полипоидной саркоматоидной карциномой, которая была обнаружена в виде большой внутрипросветной полипоидной массы. Опухоль была успешно удалена эндоскопическим методом диссекции в подслизистом слое. При гистологическом исследовании было отмечено наличие карциноматозного и саркоматоидного компонентов. При иммуногистохимическом исследовании была обнаружена связь опухоли с вирусом папилломы человека и мутацией в гене р53. Несмотря на размер >4 см, саркоматоидная карцинома была полностью удалена эндоскопически. Планируется проведение лучевой терапии и динамическое наблюдение.

Sarcomatoid carcinoma of the esophagus is a rare malignant neoplasm that accounts for approximately 2 % of all esophageal carcinomas. It is also referred to as carcinosarcoma, pseudosarcoma, pseudosarcomatous squamous cell carcinoma, fusiform cell carcinoma, and polypoid cancer, reflecting the uncertainty of its pathogenesis. The article presents a case of polypoid sarcomatoid carcinoma in a 68‑year-old patient, which was detected as a large intraluminal polypoid mass. The tumor was successfully removed endoscopically by dissecting in the submucosal layer. Histological examination revealed the presence of carcinomatous and sarcomatoid components. Immunohistochemical examination indicated a connection between the tumor and HPV infection, as well as a mutation in the p53 gene. Despite its size, over 4 cm, the sarcomatoid carcinoma was completely removed endoscopically. Radiation therapy and dynamic monitoring are planned.
